Providence is calling a Pharmacist (1.0 FTE, Day Shift) to Pharmacy Department at Providence Infusion & Pharmacy Services in Tukwila, Washington.

In this position you will:
Interpret orders and/or prescriptions for all intravenous (IV) medications and infusions to be provided for patients. Assures that the appropriate dose, rate, and route of administration have been ordered.
Analyze and assess patient laboratory values for clinical intervention and monitoring.
Clinical management of patients on infusion therapies that include: IV antibiotics, biologics, total parenteral nutrition, hydration, and patient controlled analgesia.
Oversees technicians to ensure that the final product is stable and safe to infuse intravenously.
Dispense IV solutions and ancillary supplies required for the proper infusion of the product.
Coordinate the transition of the patient from the hospital to the home setting along with the physician, floor nurse, infusion nurse clinician, and other professionals as needed.
Collaborate with an interdisciplinary team including nurses, pharmacists, registered dieticians, and pharmacy technicians.
Required qualifications for this position include:
Bachelor's Degree or PharmD from an accredited school of pharmacy
Current Washington State Registered Pharmacist License
2 years of experience as a Licensed Pharmacist
Preferred qualifications for this position include:
Infusion, long term care, or hospital pharmacy experience
TPN or biologic infusion management experience
