Providence Pharmacist - Admitting Coordinator(Patient Registration) in Tukwila, Washington
Providence is calling an Admitting Pharmacist Coordinator (1.0 FTE, Day Shift) to work in the Patient Registration Department at Providence Infusion and Pharmacy Services in Tukwila, WA.
This position serves as member of the infusion team, responding to incoming referrals and informational calls regarding outpatient / home infusion services. The Admitting Pharmacist is responsible for collecting and reviewing clinical information in order to ascertain the appropriate services for the individual referred. This position is responsible for assisting the patient, referral source and any other callers with infusion services.
In this position you will:
Process, dispense and check medication and prescriptions and assist pharmacy support personnel (pharmacy technicians).
Provide pertinent drug information to physician(s), providers and nursing staff regarding drug therapy.
Assigns and coordinates referrals to the appropriate team, taking into consideration caseload availability, geographic areas, staff skill level and expertise, and refers to outside nursing agencies as needed.
Verify prescriptions phoned in by providers or designated agent of the Medical Director against online medication profiles in the Hospice database, contacting clinicians for clarification if the order phoned in doesn’t match the data base or if coverage is in question.
Identify and Reduce medication therapy duplication.
Maintains close communications with Intake RNs and Dietitians regarding clinical needs of patient; with Hospital Liaisons, hospital based Nurse Clinicians, Hospice Coordinator and other referral sources in relationship to issues around patient referrals, discharge plans, and visit needs; and with Admission Assistants for authorizations and updates.
Required qualifications for this position include:
PharmD From an accredited school of pharmacy
2 years Experience as a licensed pharmacist.
Current Washington State Registered Pharmacist license.
Preferred qualifications for this position include:
Completion of an ASHP approved residency program
Board Geriatric Certification preferred.
