Providence is calling a Social Worker (Pace Spokane, Days, 1.0 FTE) to the Providence Elderplace in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Complete comprehensive social work assessment and periodic reassessment of participants, including mental status tool. Shares findings with other disciplines and referral agencies as appropriate.
Refer participants and their families/friends to appropriate community agencies.
Serve as a liaison and advocate with such organizations to assure participants receive full range of services to which they are entitled.
Participate in the interdisciplinary team planning conferences to develop an integrated, individualized plan of care.
Bring participant input into the care planning process.
Involve participant, family, and other caregivers in developing an individualized plan of activities, long-term goals, and short-term objectives.
Interpret fiscal assessment of eligibility and fees, if any, to participant and family/friends as needed. provide technical assistance to participants on managing finances.
Complete periodic reassessments of participants and develops care plan addenda.
In support of the primary physician and nurse practitioner, assists in discussions related to Advanced Directives and assuring that participant’ wishes are respected. Provides compassionate, high quality end-of-life care to participants.
Assist each participant, any caregivers, and all center staff in their efforts to maximize participant functioning by bringing an understanding of social/emotional factors related to health problems and each participant’s unique life history and situation.
Assist the participant and caregivers in matters related to establishment of a guardian or surrogate decision maker.
Recognize signs of physical emotional and verbal abuse. Reports suspected abuse and neglect to appropriate state agencies for intervention. Apprises interdisciplinary team of outcome of investigations.
Coordinate needs during transition periods with participants, family and PEPS staff. Assist in placement of participants in long term care facilities and maintain contact with participants to assure continuity of care.
Responsibilities to Multidisciplinary Team: Maintain communication with the Social Services Manager concerning caseload issues and develop processes to provide social work coverage during absences. Maintains close working relationships with center supervisors, physician and clinic staff, rehab staff, and personal care aides.
Counseling/Education: Provide individual and family counseling. develops and lead group counseling and other needed support activities, including regularly scheduled classes or groups for cognitive stimulation, enrichment, and/or emotional support. Provides individual and family education to maximize adaptive behaviors related to coping with aging process. Handles unexpected events and possible crisis intervention flexibly and creatively. Offers grief and bereavement support to participants and caregivers. Provides support services needed to address concerns of participants with a history of substance abuse.
Administrative/Record-Keeping: Maintain current written social work records and meet all administrative requirements, e.g., statistical records and service reports required monthly, quarterly, and annually. medical records. staff meetings. Maintains all participant information and records in a professional and confidential manner, per HIPAA regulations.
Required qualifications for this position include:
Master’s degree in Social Work from an accredited school of social work or equivalent education/experience
Washington State Agency Affiliated Counselor or LIACSW or LAASW or LICSW or LASW
Current Cardiopulmonary Resuscitation (CPR) certification
Valid Washington State driver’s license, and automobile insurance
One (1) year work experience with the geriatric population
Two (2) years social work experience including effective work with geriatric age groups from diverse ethnic cultures
