Providence RN Admissions/Discharge - TCU (Transitional Care Unit), Day Shift, Full-Time - Spokane, WA *Up to $10,000 Hiring Bonus* in Spokane, Washington


Providence is calling a Admission/Discharge RN (Sub Acute Care I, Day Shift, 1.0 FTE) to Providence St. Joseph Care Center in Spokane, Washington. In this position you will:

  • Coordinates new patient admissions and discharge from facility processes including: Nursing assessments, admission documentation, discharge documentation, and provision of patient education. Maintains confidentiality of all patient demographics, medical and financial information at all times. Ensures proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.

  • Administers initial comprehensive nursing assessment and evaluation upon admit to unit based on the nursing process. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application.

  • Collaborates with MDS team to ensure compliance with regulations, PDPM, and other required elements are completed.

  • Contributes to the total plan of care for the participates/residents/patients/clients by creating baseline Care Plan, making pertinent nursing observations and participating in multidisciplinary team care conference as needed.

  • Maintains regular and ongoing communications with family members, nursing staff, interdisciplinary team members and others to ensure 24-hour continuity of care and unified approach by the team for each participants/residents/patients/clients.

  • Maintains appropriate documentation and legal clinical records according to established guidelines.

  • Participates in survey inspections by authorized government agencies including completion and delivery of any requested correspondence.

  • Delegates aspects of nursing care to Nursing Assistants to ensure continuity of care, and assists in orientation and development of new employees on the team.

  • Provides relief supervision on all units for weekend call rotation with members of Nursing Leadership, and in the absence of Nurse Manager or the Director of Nursing.

  • Demonstrates awareness of legal issues, including confidentiality pertinent to patient care/unit function and actively supports risk management activities.

  • Attends staff meetings, in-service meetings, and participates in agency committees or task force activities, as assigned. Participates in standards development/quality assurance activities.

  • Complies with all applicable Federal, State, and local laws, regulations, and requirements as well as facility specific policies and procedures. In addition, all employees who have contact with participants/residents/patients/clients are expected to promote the Participant/Resident/Patients/Clients Bill of Rights and Responsibilities and understand the basic procedures for receiving and documenting grievances in order to initiate the appropriate process for participant concerns.

  • Adheres to all policies and procedures including requirements with regard to infection control, practicing universal precautions, sanitation, and safety and disaster preparedness.

Required qualifications for this position include:

  • Education sufficient for licensure

  • Current licensure as a Registered Nurse in the state of Washington

  • Current CPR certification or must obtain within 30 days of hire

  • 1 year experience in the SNF/LTC environment

Preferred qualifications for this position include:

  • Experience as an Admission Nurse, or Charge Nurse experience in a SNF/LTC setting

Our Transitional Care Unit is designed to support patients transitioning from the acute hospital who need skilled nursing and/or therapy services to return home or help with placement in the community with our many local partners. We offer full nursing services including: IV therapy, wound care, diabetic and medication management. PSJCC also serves the needs of Long-Term Care residents offering a residential setting supporting a resident-centered model.
