Providence Health & Services ADH Case Manager/LPN in Spokane, Washington
Providence is calling a Licensed Practical Nurse (LPN) to support Spokane Adult Day Health (0.0 FTE, Days) at Providence ElderPlace in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Assists with, or performs according to scope of practice, client medical care activities/treatments, according to policies, procedures, and protocols.
Considers individual needs in the assessment and treatment of clients, including ethical, cultural, psychosocial, religious, and age-specific aspects, for impaired adults and the elderly.
Monitors clinical status for patients ensuing that they are being cared for at the appropriate level of care.
Assists with, or performs, according to scope of practice, a timely assessment of clients, according to policies, procedures and protocol, taking into account age/developmental stage.
Assists with care planning; writes multi-disciplinary treatment plan of care.
Assists with quarterly and annual care plans.
Maintains confidentiality of highly sensitive patient, medical staff and administrative information. Performs all activities with discretion.
Documents care given and contacts made.
Shares accountability for medication storage and distribution.
Maintains a safe environment for clients, staff and visitors.
Communicates effectively to clients, staff and visitors.
Understands and follows the center's policies.
Fills in and covers duties of other nursing and case management staff as needed.
Performs other duties and responsibilities as assigned.
Required qualifications for this position include:
A Licensed Practical Nurse from an accredited school of nursing
Experience using computers and data entry
Requires a current Washington State Licensed Practical Nurse license
Preferred qualification for this position include:
- 1 year of experience working with elderly
