Providence Health & Services RN Case Manager - West Seattle in Seattle, Washington


Providence is calling a Registered Nurse (RN) Case Manager (PACE Program, Day Shift, 1.0 FTE) to Providence ElderPlace in West Seattle, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

Offering $5,000 Sign-on bonus for external hires! Monday-Friday; no weekends or holidays.

The Registered Nurse Case Manager establishes a well rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities.

In this position you will:

  • Acts as a medical advocate for the participant, and coordinates care services with the Interdisciplinary team; Determines whether care is warranted or if IDT needs to work with family and providers to ensure understanding of benefits and risks of procedure and treatments.

  • Understands and employs the Essential Activities of Case Management: Assessment, Planning, Implementation , Coordination, Monitoring and Evaluation of disease management in maintaining our participants in the community.

  • Builds strong relations with participants, caregivers, families and community providers so that communication and coordination occurs in a manner that facilitates optimal care and financial oversight.

  • Conducts initial and ongoing nursing assessments of all participants; monitors health problems and health status as needed, and implements the nursing care plan.

  • Ensures that documentation reflects adherence to nursing standards, interdisciplinary care plan and progress toward identified patient outcomes.


Required qualifications for this position include:

  • Graduate of accredited school of nursing

  • Current Washington State Registered Nurse (RN) license

  • Current Cardiopulmonary Resuscitation (CPR) certification

  • Current Washington State drivers license

  • Proof of auto insurance

  • One (1) year of long term care or geriatric nursing experience

Preferred qualifications for this position include:

  • Bachelor Nursing Degree

  • Current CCM, NACCM, ACME

  • Medical Surgical Experience or Rehabilitation Medicine
