Providence System Executive Director Care Management in Renton, Washington

Description

THE ROLE

The system Executive Director of Care Management is responsible for leading, setting strategic direction, coordinating and supporting care management activities for all inpatient care management functions. The system ED-CM works in partnership with Medical, Nursing, Quality, Program, Home and Community Services, Utilization Management, and external partners across the system to achieve optimal patient care, and attainment of organizational goals.

This position has accountability for inpatient (IP) care management services to include: care coordination, social work, transition planning, and patient care progression. This role will lead as a SME in this space and work across the entire continuum of care to establish best practices, reduce variation, modernize, automate functions, support new models of care and deliver on our commitment to our patients of best outcomes and experience.

Studies on an ongoing basis national and regional trends and adjusts the care management model to meet financial, regulatory and clinical quality goals for the system. In collaboration with the analytics team, develops, monitors, and reports on most relevant metrics/dashboard in this space and consistent with organizational goals to move toward value-based care.

ESSENTIAL FUNCTIONS

  • Provides leadership and strategic planning to operationalize standard care management services and develops a coordinated approach to care management across the continuum, including the development of a post-acute care strategy.

  • Oversees development of goals, creation and implementation of action plans and program/intervention evaluation.

  • Facilitates collaboration and coordination among all divisional entities to develop and facilitate best practice(s) in care management and care coordination.

  • Facilitates the delivery of safe, high quality care through clinical and non-clinical care management/coordination, ongoing process evaluation, and reviewing clinical outcome metrics.

  • Works with division CM leaders, CMO and CNO to evolve the organizational model. This includes being a resource and SME in the development, implementation and monitoring of annual capital/operating and personnel (FTE) budgets for the division leaders in CM.

  • Responsible to continuously evaluate and advance the inpatient Clinical Care Management model assuring best use of RN, SW and non-professional staff skill sets; proactive identification of and intervention with patients; continuity of assignments whenever possible and adequate Care Manager and Social Work coverage for all inpatient and Emergency Department settings.

  • Develops strategies to leverage resources to advance professional growth, spread best practices, and provide flexible staffing options. Convene, lead and/or monitor divisional work groups as indicated to address opportunities.

  • Ensures that all aspects of federal, state, local and The Joint Commission/DNV compliance care management related criteria are met.

  • Measures, analyzes and reports key metrics related to care management across the care continuum. Obtains, utilizes and leverages existing data for improvement of patient flow and other key performance indicators. Facilitates development and distribution of reports as appropriate.

  • Models and fosters an environment of professionalism and compassionate care. Guides the recruitment, retention, and employee engagement that keeps high performers in place and maintains focus on succession planning. Acts as a role model through the demonstration of personal professional development, continuous learning and competency improvement.

  • Serves as liaison for system wide care management-focused initiatives.

  • Provides on-going communication to key stakeholders to ensure the required support is provided for initiatives to be successful and identified issues to be addressed.

  • Influence information technology solutions at service areas, Division, and System level to support cross-continuum care management and coordination (i.e., Epic functionality, Emergency Department Information Exchange system).

  • Identifies and implements best practices related to: Clinical Care Management, Utilization Management, Chronic Disease Management, Health Promotion, Care Transitions and Transition Planning, Post-acute Services, Social Work, Hospital Throughput, Patient Placement, LOS Management, and other related areas as indicated.

  • Improve our ability to appropriately measure the impact of care models and interventions on individual and population health objectives and guide resource allocation.

  • Develops strategic priorities, tactics, business plans, and budgets as required.

IDEAL QUALIFICATIONS

  • Bachelor's Degree in Nursing

  • Master's Degree in Nursing or Business Administration or Equivalent; or equivalent educ/experience

  • Upon hire: Alaska Registered Nurse License (Vendor Managed); or California Registered Nurse License (Vendor Managed); or Montana Registered Nurse License (Vendor Managed), or New Mexico Registered Nurse License (Vendor Managed), or Oregon Registered Nurse License (Vendor Managed), or Texas Registered Nurse License (Vendor Managed) , or Washington Registered Nurse License (Vendor Managed), or National Registered Nurse Compact License (Vendor Managed)

  • Upon hire: Case Management Certification

  • 10 or more years of experience working in Acute care case management, Utilization Review, in a complex setting and at a large scale (over $2 Billion revenue sites)

  • 10 or more years of experience in progressively responsible roles with demonstrated leadership experience and strong team management skills

  • Experience in a continuum of care setting beyond acute

  • Knowledge of the healthcare industry, with a focus on population health strategies and implementation of data-driven interventions

  • Care management strategic and operational leadership with demonstrated results in the recent past

  • Proven ability to drive change in a complex organization by using effective influence without authority, and bringing together and achieving consensus among diverse stakeholders

  • Ability to address issues through innovative solutions to improve the patient experience, affordability, and quality

  • Ability to communicate a vision, lead and persist through transition processes within a complex, matrixed organization

  • Ability to establish organizational structures and management systems to achieve administrative and clinical integration across the continuum of care

  • Broad working knowledge of clinic operations and physician group practices, financial management, cost analysis, information systems, payment mechanisms and insurance contracts

  • Broad knowledge and understanding of the trends and changes taking place in health care and their implications

  • Demonstrated ability to work with medical leaders

  • Excellent problem-solving skills

  • Excellent written and verbal communication skills, including active listening abilities

  • Problem solver who involves others in key decision but assures timely decisions are made

  • Ability to establish relationships with diverse groups and mentor and develop others
