Providence is calling a Navigator Resident Services - Irvington Assisted Living Facility (1.0 FTE, Day) to Elderplace Irvington Village in Portland, OR

We are seeking a Navigator Resident Services - Irvington Assisted Living Facility that will work from an empowerment model; their primary goal is to assist the residents to live to their full potential and develop one's alternatives. This position supports residents to do as much as they are capable of doing themselves. The HSA assists residents who have requested support in enhancing the quality of their daily lives, and in more fully and successfully participating in the community at Irvington Village.

In this position you will have the following responsibilities:

  • Coordinate services that residents use, need and want to include but not limited to Mental Health, Hospice, Home Health (PT, OT, SLP, RN) wound clinic, supplies, transportation, etc

  • Assess residents' interests, participation and satisfaction with educational and social programs available to them and barriers to participation

  • identify and assess individual resident needs when appropriate; inform residents about service availability, how to apply for services and benefits, consumer rights and other relevant issues

  • Work with Irvington Life Enrichment coordinator and resident care supervisors, to promote or organize programs on topics of interest to residents

  • Assist residents in forming or strengthening resident organizations, including Resident Council

Required qualifications for this position includes:

  • Bachelor's Degree

  • 1 year of experience working with people of low income, or 3 years of documented, successful experience in community development or community outreach activities, or ALF/RCF Lead aide or Med aide experience

  • Demonstrated experience in successfully working with diverse populations

  • Experience with community outreach social service system

  • Experience working in an urban ALF/RCF

  • In compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position based upon the location of the position fingerprinting

Preferred qualifications for this position include:

  • Program evaluation experience
