Providence is calling a Licensed Practical Nurse (LPN) (Long Term Care, Variable Shift, Per Diem) to Providence Mother Joseph Care Center in Olympia, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Administers total, comprehensive care to an assigned caseload based on the nursing process and the interdisciplinary care plan.
Conducts initial and ongoing nursing assessments of all participants/residents/patients/clients. monitors health problems and health status as needed, and implement nursing care plans.
Maintains shift accountabilities and ensures continuity of care through proper communication and documentation.
Initiates a patient education plan according to the individualized needs of the patient, in conjunction with the Primary Care Physician.
Provides health counseling and re-instructs participants and family members as needed.
Required qualifications for this position include:
Education sufficient to qualify as an LPN.
Current CPR certification or must obtain within 30 days of hire.
Preferred qualifications for this position include:
One (1) year of LTC geriatric experience
One (1) year of experience as a staff nurse with LTC experience.
Supervisory experience with nursing assistants.
Experience with an interdisciplinary team.
(MDS) Minimum Data Set/ (RAI) Resident Assessment Instrument experience.
