We are seeking a Hospice, Home Health Aide that will be under the supervision of a registered nurse, provides supportive personal care services and homemaking assistance to patients of all ages according to the individualized physical needs of the patient in accordance with state and federal regulatory requirements.
In this position you will have the following responsibilities:
Actively support and incorporate mission into daily activities as outlined in the Department Mission Practicum
Perform all duties and functions under the direction of a Registered Nurse, in cases of Therapy only patient in Home Health, the aide will also receive direction for care from the Therapist
Demonstrate positive interactions with the public, and positive interpersonal relations in dealing with fellow employees, supervisors, and other department employees, so that productivity and positive employee relations are maximized
Report to work as scheduled and on time per Attendance and Punctuality policy
Maintain confidentiality of all information related to patients, medical staff, and as appropriate, other information
Support and contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities
Encourage and/or assist the patient to become as independent as possible within medically determined limitations and promotes general rehabilitation through ambulation and exercise as prescribed
Provide or assists with personal care, bathing, toileting, feeding, oral hygiene, hair and nail hygiene and other activities of daily living
Perform simple procedures as an extension of appropriate therapy services; such as transfer techniques, ambulation skills, prescribed exercises, etc., which have been taught by the professional
Assist with oral medications that are ordinarily self-administered
Perform non-sterile dressing changes and administers simple enemas for selected patients, as instructed by the registered nurse
Review diet as planned and instructed by appropriate personnel
Simple meal preparation, including special diets
Do light housekeeping necessary to maintain healthful and safe environment
Observe deviations from usual physical or mental status and reports to registered nurse or supervisor, (This may include taking TPR and BP in selected cases)
Initiate a positive flexible problem solving
Encourage and/or assists the patient to become as independent as possible, determine limitations and promotes general rehabilitation through ambulation and exercise as prescribed and patient's condition allows
Record information in patients clinical record to document service given and observations noted to meet hospital standards; state, federal and JCAHO regulations
Meet mandatory federal requirement of twelve (12) hours in-service per calendar year
Qualifications:
Required qualifications for this position includes:
Current Oregon Certified Nursing Assistant (CNA) certification
Current Basic Life Support (BLS) for the Healthcare Provider certification
Valid driver's license
Two (2) years experience in a health care setting providing hands on patient care, -OR- Nursing student with current CNA
