Providence is calling a Chaplain Hospice (0.6 FTE, Days) to Providence Medford Medical Center in Medford, OR
This opportunity has a Sign on bonus of $2,000, that will be paid out in 2 installments over the span of 12 months from your start date.

We are seeking a Chaplain Hospice that will be under the direction of the Director of Pastoral Care and in conjunction with the Hospice Program Manager, the Hospice Chaplain provides Pastoral Care to Hospice patients, families and hospice staff.
In this position you will have the following responsibilities:
Provide pastoral care services to Hospice patients and families, and to staff as requested
Provide spiritual support and counseling, as appropriate, to patient and family, through telephone calls, home visits, nursing home and hospital visits
Provide spiritual support and counseling to staff as requested
Pray weekly for Hospice patients, families, staff, and the larger community
Assist families with funerals or memorial services as requested
Work closely with Hospice Social Worker to provide bereavement support and counseling to patients and families
Respect confidentiality of all patient information and care team discussions
Maintain a file of spiritual resources on death, dying, and bereavement
Communicate with other local clergy
Coordinate referrals to community clergy or pastoral counselors
Initiate and maintain contact with patient's and/or family's pastor, priest, rabbi, or faith community as appropriate
Work closely with patient's clergy and hospital chaplains whenever possible
Communicate with both Home Health/Hospice and Pastoral Care Department staff members
Participate in weekly Hospice care team meetings (IDG), helping assess patient/family spiritual needs and contributing to care plan
Participate in monthly bereavement care team meetings
Attend monthly Hospice staff meetings
Notify DPC of any unusual situation, which could become a public relations or ethical issue
Serve on the Hospice Ethics Committee
Assist other staff with education and community awareness
Assist other staff and Chaplains in assessing educational needs of local clergy on matters of death, dying and bereavement
Qualifications:
Required qualifications for this position includes:
Master's Degree in Theology, Pastoral Ministry -OR- related field
Coursework/Training: Successful completion of four units (minimum) of Clinical Pastoral Education
Certification by either the NACC (National Association of Catholic Chaplains), APC (Association of Professional Chaplains), NAJC (National Association of Jewish Chaplains), -OR- CASC (Canadian Association for Spiritual Care) or obtainable within two (2) years of hire
Certification will be maintained in 'active status' as verified by the department Director of Mission Integration and Spiritual Care for Home & Community Services
Must have endorsement from sponsoring religious community
Current drivers license
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
Current BLS (Basic Life Support for Health Care Provider) -OR- ACLS (Advanced Cardiac Life Support) within thirty (30) days of hire
Preferred qualifications for this position include:
- Pastoral Care chaplaincy in a healthcare setting with bereavement and/or hospice experience
