In this position you will:
Admission, recertification, and discharge assessments.
Create and modify care plans
Point of care documentation
Assess wounds and ostomies and make recommendations to the RN case manager for optimal wound and ostomy management.
Collaborate with the care team and providers
Obtain orders for services and enter in the EMR
Provide information to patients & significant others regarding diagnosis, interventions & available resources.
Demonstrate understanding of and exhibit support for organizational behavior guidelines.
Required qualifications for this position include:
Graduate of an accredited school of nursing.
2 years nursing experience, including one year direct-patient experience with wound certification
Current Washington State Registered Nursing License
Valid Washington State driver’s license
Insured and reliable transportation
Current Wound Certification (CWOCN, WOCN, CWCN, WCC, CWS, CWCA)
Healthcare Provider BLS/CPR certification
Preferred qualification for this position include:
1 year Home health care experience
Prefer experience with wound certification in an inpatient or clinic setting
