Providence Medical Social Worker - Kodiak Hospital and Long-Term Care in Kodiak, Alaska
Description
The Social Worker MSW is a split position that works with both hospital patients and long-term care residents. In the acute hospital setting the social work role is fast-paced and focused on providing resources, and skilled support to allow patients to discharge safely to the next level of care. The long-term care side offers opportunities to form longer-term relationships as the social worker helps people adjust to and thrive in the LTC setting, and cope with changes and loss. Both settings require great organizational skills and team work as well as an ability to be a strong patient advocate. They require flexibility and creative problem-solving within the unique Kodiak Island Community.
The social worker is a member if on-island multidisciplinary teams for daily collaboration, and a regional team for social-work specific guidance and support. The Social Worker MSW works under the direction of Care Management Leadership and is employed by Providence Health & Services. The position will practice in accordance with competencies and guidelines outlined in Standards for Social Work Practice in Health Care Settings and the Code of Ethics as defined by the National Association of Social Workers.

Hours: Full-time, 1.0 FTE, Day Shift, 40 Hours Per Week
$10,000 Hiring Bonus for eligible external hires that meet required qualifications and conditions for payment.

Required qualifications:
Master's Degree Degree as a Social Worker from an accredited School of Social Work.
Upon hire: Licensed Master of Social Work Or
Upon hire: Licensed Master of Social Work Temporary
Preferred qualifications:
Certification in Oncology Social Worker (OSW) in Oncology Setting
American Case Management Certification (ACM) in Acute Care Setting
2 years Medical Social Work experience.
Experience with computerized documentation and medical records is preferred at the time of hire.
