Clinical Manager Hospice - Home Services Administration at Providence Home Services Oregon

This position serves as the manager of a clinical program and provides staff supervision and coordination for the care provided to patients and their families served by a designated hospice interdisciplinary team. This position is responsible for identifying 'best practices' for care. As well, this position may network with community resources and consult with community health care providers serving patients with life threatening illness.

Required qualifications:

  • Bachelor's Degree in Health care related field -OR- Master's Degree in Social work or spiritual care

  • Current licensure in the applicable discipline (e.g., Nursing, Physical Therapy) required in the state of Oregon. Staff who provide service in the state of Washington require Washington state licensure in the applicable discipline.

  • Current driver's license and automobile insurance.

  • Current Healthcare Provider CPR certification

  • Three (3) years of Clinical experience in chosen field

Preferred qualifications:

  • Hospice certification or advanced certification

  • Hospice experience

  • Three (3) years of Staff supervision experience

  • Proficient in using computers and Microsoft Office Suite programs required; knowledge of EMR
