Providence RN After Hours Triage Per Diem (Work from Home) - Hospice Snohomish in Everett, Washington

Description:

Providence is calling a Registered Nurse After Hours Triage (Per Diem, Variable Shift) to Hospice Snohomish at SCS Hospice in Everett, Washington. Primarily work from home.

This is primarily a work at home position triaging patient/family phone calls, consulting about end-of-life pain and symptom mangement. There will be times where home visits need to be done while on shift. The Hospice Nurse serves as a member of the interdisciplinary hospice team, assessing, planning, and providing nursing care to terminally ill patients. In doing so, the nurse addresses, in a holistic way, the needs of terminally ill patients and their families. The nurse strives to anticipate changes in patients' conditions, avoid crises, promote patient and family choice, assure continuity of care, and enhances the quality of life of dying patients.

In this position you will:

  • Assess the needs of Hospice and families by phone, with special attention to the symptoms and comfort of the patient; provide immediate problem-solving support.

  • Contact covering MD’s to obtain needed medications, arrange for nursing visit if needed, and/or provide follow-up communication to other Hospice staff regarding patient/family needs.

  • Implement nursing interventions based on the Plan of Care and Providence policies and procedures.

  • Anticipate disease progression and implications for pain and symptom management at end of life.

  • Inform patient and family of current and anticipated changes in patient condition as appropriate.

  • Evaluate and document patient and family response to interventions, revise the Plan of Care as appropriate

  • Document RN assessments, interventions, and communications accurately and timely according to established standards

  • All required documentation will be complete, accurate and timely. Be able to document nursing assessments and interventions related to symptom management and palliative care, grief and quality of life issues for patient/families receiving Hospice care. Documents in electronic health record.

  • Promote and practice patient/family, personal and environmental safety while performing job duties according to agency policies and procedures.

  • Participate in relevant educational activities to provide competent care to patients/caregivers, to improve the nursing process, and to seek personal and professional growth.

  • Display professionalism in the work environment in all areas including communication with both internal and external customers and time management skills.

  • Maintain productivity as per agency standard.

Qualifications:

Required qualifications for this position include:

  • Graduate of an accredited school of nursing

  • Current Washington State Registered Nurse (RN) License

  • Current provider BLS (Cardiopulmonary Resuscitation) certification

  • Driver auto insurance

  • Driver's license

Preferred qualifications for this position include:

  • One (1) year of nursing experience

  • Medical/Surgical, Hospice or Home Health experience
