Providence Health & Services Physical Therapist - Home Health Snohomish in Everett, Washington
Providence is calling a Physical Therapist (PT) (Home Health Snohomish, Day Shift, 0.0 FTE) to SCS Home Health in Everett, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
The PT will perform comprehensive assessments to determine specific therapy needs, develop plans of care, coordinate care with other disciplines and provide direct patient care in the implementation of outcome oriented physical therapy interventions under the physician ordered Plan of Care.
All required documentation will be complete, accurate and timely.
Promotes and practices patient/family, personal and environmental safety while performing job duties according to agency policies and procedures.
Participates in relevant educational activities to provide competent care to patients/caregivers, to improve the rehab process, and to seek personal and professional growth.
Displays professionalism in the work environment in all areas including communication with both internal and external customers.
Maintains productivity as per agency standard.
Required qualifications for this position includes:
Bachelor's Degree In Physical Therapy from an institution approved by the state board.
Current Washington State Physical Therapist License.
Current Washington State driver’s license and have access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.
Current Provider CPR certification.
Proof of current Auto Insurance.
Preferred qualifications for this position includes:
- 1 year Therapy with adult physical disabilities
